Employment Forms

If you have been instructed to download the Statement of Personal History form, you have two options:

1. The preferred option is that you download the SPH PDF FORM, which will allow you to fill out, save and submit the form via personal computer (this form can also be printed and mailed, but we would prefer that you use the built-in “submit” button on the document itself.  You must have Adobe Reader installed on your personal computer.

 

1. The second but less desirable option would be to download the SPH MS WORD FORM, which you can fill out, save and mail to the address provided in the form instructions.

Please read the instructions in the SPH documents carefully before filling in and submitting your form.

Your ability to follow instructions accurately and honestly is part of the background process.
