INSTRUCTIONS FOR SUBMITTING THE SPH FORM
Forms may only be submitted during the open recruitment period in one of the following two ways:
- By Email: Save and attach this form to an e-mail and send it to firstname.lastname@example.org
- If you are submitting the SPH via email, please provide it either in a .pdf, .doc or .docx file.
- Please DO NOT submit the form in a .pgs or .pages extension.
- In-Person: Print out the completed form and drop it off at C-COM at 2200 Kaen Road in Oregon City between the hours of 7a-5p Monday through Thursday ONLY.
You may expect to receive contact from Meg Gilliland within 5 business days of the receipt of your SPH. If you have not heard back within 5 business days, please email Meg at email@example.com
DO NOT submit or turn in this SPH form if it is not complete.
